Chronische pijn heeft meestal geen fysieke oorzaak: Zo hebben tweederde van de mensen zonder pijn allerlei structurele afwijkingen – en veel mensen met chronische pijn geen medische verklaring voor de pijn. Het pijnsignaal komt uit het brein en is bij chronische pijn geconditioneerd in het lichaam. Als onbewust, automatisch patroon, onafhankelijk van fysieke structuren.

Uit onderzoek door middel van MRI scans van 98 willekeurige mensen zonder rugpijn blijkt dat 64% (van deze mensen zonder rugpijn) onder andere hernia’s en bulging discs (uitstulpingen) hebben. Ook had 38% afwijkingen in meer dan één tussenwervelschijf. Daarbij had 8% slijtage in de wervels. Maar niet één van deze mensen had pijn. Dat betekent dat er geen oorzakelijk verband hoeft te zijn tussen chronische rugpijn en structurele afwijkingen.

Results

  • The prevalence of bulges, but not of protrusions, increased with age.

Conclusions
On MRI examination of the lumbar spine, many people without back pain have disk bulges or protrusions but not extrusions. Given the high prevalence of these findings and of back pain, the discovery by MRI of bulges or protrusions in people with low back pain may frequently be coincidental.

Conclusions: The impression obtained from an MRI scan does not determine whether lumbar stenosis is a cause of pain. Electrodiagnostic consultation may be useful, especially if age-related norms obtained in this study are applied.

De meeste chronische pijn wordt op middelbare leeftijd ervaren

Omdat de meeste chronische pijn op middelbare leeftijd wordt ervaren is de slijtage, waarnaar vaak wordt verwezen als oorzaak van chronische pijn, niet de werkelijke oorzaak. Als dat wel zo zou zijn zou de pijn immers voorspelbaar toenemen – naarmate de artrose voortzet met de leeftijd – in plaat van afnemen.

De meeste pijn wordt ervaren tussen de 30 en 60 jaar. De periode waarop er de meeste verantwoordelijkheid, stress en zorg om carrière en gezin. Na de 60 is er dan ook een duidelijke afname van chronische pijn.

Naarmate de leeftijd verder toeneemt neemt ook de slijtage toe. Dit hoeft geen pijnervaring op te leveren. Chronische pijn neemt meestal toe na operaties

Onderzoeken naar de effecten van operaties bij chronische pijn

Opereren maakt chronische pijn vaak erger

Bij operaties is de kans groot dat de chronische pijn verder toeneemt. Dit blijkt uit een review van 25 klinische onderzoeken. Slechts bij een kleine minderheid neemt de pijn tijdelijk af. Uit de meta-analyse blijkt dat de placebo-operatie (waarbij alleen een incisie en hechting wordt uitgevoerd) zelfs beter scoort dan de echte operatieve ingreep. Dat impliceert dat chronische pijn geen fysieke oorzaak hoeft te hebben, maar een neurologisch probleem is. Ofwel een mindbody probleem. Dat met een integraal mindbody proces moet worden opgelost. Daarbij toont het aan dat een invasieve behandeling de algehele sensitisatie (gevoeligheid) vergroot, waardoor de intensiteit van de pijn toeneemt.

Verschillende onderzoeken naar de effectiviteit van een mindbody poces – zowel ISTDP (intensive short-term dynamic psychotherapy) als CBT (cognitive behavorial therapy) bij chronische pijn geven aan dat een gericht psychogeen (mindbody proces) effectief is bij chronische pijn. Dankzij de langdurige resultaten en relatief korte behandelperiode is het een kosteneffectieve manier om chronische pijn te bestrijden.

  • Clinical- and cost-effectiveness of intensive short-term dynamic psychotherapy for chronic pain in a tertiary psychotherapy service, 2020

RESULTS:

Multilevel models indicated significant reductions in symptoms and interpersonal problems during treatment, including a moderate to large pre-post effect size (d = 0.76) for somatization. Further, the sample had successive reductions in yearly healthcare costs, reaching the normal population mean two years post-treatment.

CONCLUSION:

Within the limitations of the uncontrolled design, our study suggests that ISTDP may be both clinically effective and cost-effective for patients with chronic pain.

  • A Pilot Three-Armed Randomized Controlled Trial Comparison with Mindfulness-Based Stress Reduction, 2016

Results: In the intention-to-treat analysis, the ISTDP and CBT groups both showed improvement
in the primary outcome after treatment. Pain symptoms in both conditions were significantly
reduced. Both ISTDP and CBT groups demonstrated reductions in psychological distress,
depression and catastrophic thinking, and also increases in the use of relaxation as a coping
strategy. The CBT group showed an improvement in self-efficacy that was not obtained in the
ISTDP group. Also, significant differences were not observed in the primary and secondary
outcomes at the three-month follow-up compared to post-treatment. Overall, both treatments
were equally effective at the three-month follow-up.

Conclusion: Our results suggest that ISTDP may provide an effective alternative therapy for
medically unexplained somatic symptoms of pain.

Our integrative treatment involves delivering a progression of interventions, as needed, to achieve pain reduction: tailored pain neuroscience education, cognitive and mindfulness skills to decrease the pain danger alarm mechanism, behavioral engagement in avoided painful and other feared activities, emotional awareness and expression to reverse emotional avoidance and overcome trauma or psychological conflict, and adaptive communication to decrease interpersonal stress.
